Chronic Anemia as First Clinical Manifestation of a Prolactin-secreting Pituitary Macroadenoma in a Male Patient
A 58-year-old man was studied for mild chronic anemia of 6 years' duration. Familial and personal history of the patient was irrelevant and he took no medication. He complained only of erectile dysfunction for the past 10 years. Initial blood count showed a mild normocytic and normochromic anemia (hemoglobin 12.1 g/dL, normal range [N] 13.0-17.0; hematocrit 34.2%, N 40-54; mean corpuscular volume 89.4 fL, N 82-98; mean corpuscular hemoglobin [MCH] 31.0 pg, N 27-31; MCH concentration 35.5 g/dL, N 32-37, with normal white blood cell (4600/mm3, N 4000-11,000) and platelet (123,000/mm3, N 140,000-450,000) counts. SummaryCriteria for the introduction of new vaccines into routine public health practice are becoming increasingly stringent. For vaccines that are expensive and those that provide moderate protection, the ability to confer herd protection could be crucial to policy deliberations about vaccine introduction. Traditionally, herd protection has been assessed after a vaccine is introduced, delaying the availability of data on herd effects to inform decisions about vaccine introduction.
